I like to start the year with a back-to-school survey. This particular survey is for secondary students, and can be customized to your needs. You can conduct this however you want: online or on paper, but I think this survey is a great way to get to know your students. I use this survey to figure out the needs and wants of each student, and try my best to tailor the lesson plans and classroom activities to my students.
